How We Care

At Reasons Health you are not a “corporate covered life”, a “file number” nor an “appointment”. You are a person. You have different concerns and needs than others. We care for your whole self which means your mental and physical health, your emotional health, your social health and your spiritual health. We listen. We care plan together using best practices and modern technology.

Step 1

Make an appointment

You can make an appointment in a way that is convenient to you: schedule an appointment by calling 779-356-4095. You will be sent an intake form that is a very important part of your care. Please return this to us prior to your visit or bring it with you on the day of your appointment. It is very important that you complete this form because it helps us understand your situation better.

Step 2

Your first visit

Your first visit lasts approximately one hour. We listen to your story. We will review your intake form together. We ask a lot of questions about you including your medical history, your family history, current and past medications and your symptoms. We discuss possible diagnoses. There are occasions when establishing a diagnosis occurs over time. We talk about the diagnostic process and why we think a diagnosis applies to you. In some cases, we talk about why a diagnosis may not apply to you. Most importantly, though, you are absolutely involved in the process every step of the way.

Step 3

Developing your plan of care

During your initial appointment we put together a plan of care that will address your whole self. A plan of care includes management of your symptoms which may, or may not, involve medications. It also includes your physical health, your emotional health and your social health. If you wish, it will include your spiritual health. A plan of care must suit your goals and your lifestyle. In other words, it has to work in your life.

Step 4

Your follow up care and beyond

Follow up appointments last approximately 30 mins. We review your plan of care and it’s effectiveness. We talk about your symptoms. If something isn’t going the way we want, we adjust the plan as necessary. The frequency of follow-up visits depends on how you feel. You will have access to your patient portal for online scheduling.

Meet the team

Tracey Odishoo, MSN, FNP, CNP, BC

My name is Tracey Odishoo. I am a family nurse practitioner and founder of Reasons Health. I created Reasons Health because our community is in great need of psychiatric/mental health providers. I have the had the privilege to live in McHenry County for 30 years; I have had the honor of caring for so many exceptionally wonderful people. I want to extend my gratitude to my community increasing access to psychiatric/mental health care.

I have been a nurse for 30 years. I obtained my bachelors degree in the science of nursing at Barat collage I received my graduate degree in public health nursing with a clinical track in family practice at the University of Illinois in 2001. My career path initially took me to emergency services where I found my true passion for helping the mentally ill and the homeless. In turn, I focused my interest in psychiatry/mental illness. I have spent the past several years caring for community members who have been hospitalized due to their mental illness. In addition, I have provided cared for those who needed PHP and/or IOP as well as outpatient care.

I have been married to my high school sweetheart for over 35 years. Together we raised three children who gave us a combined 7 amazing grandchildren! We have two cats who pay attention to us sometimes. I love music, reading and plants and flowers.

Referral Services

We have a community network for referral services, if needed, which includes PCP, medical specialist, specialized therapy, neurology and neuropsychological evaluation. We also have a vast network of social service referrals.

Medication Management

Reasons health works with multiple pharmacies for e-prescribing for your convenience.

Long-acting injectable medication is available for appropriate conditions.

Genetic testing is available.

Telehealth

Although we enjoy seeing you in person, telehealth is available. First, though, we want to get to know you much better and we want you to feel much better. Initial visits are in person. Subsequent telehealth depends on how well you feel and how well you seem. From here, we will figure out a telehealth schedule as we go.

Reasons Health does not engage in prescribing medications that contribute to addiction. We do not provide nor prescribe benzodiazepines, opioids, opiates or other narcotics or narcotic-like medications or scheduled ll medications such as Adderall or Vyvanse etc.

Reasons Health believes that certain medications, such as Suboxone, save lives.

Appointments are available for those who have been on Suboxone maintenance therapy continuously for at least one year. Verification of continuous maintenance therapy is required via IL PMP or by other means as necessary. Monthly visits and drug testing are required for continuous maintenance. For initiation or reinitiation of Suboxone we are happy to provide referrals.
